Who we are

Fazeel Pharmacy, 30 Northland Drive, Glasgow, G14 9BB, is a community pharmacy registered with the General Pharmaceutical Council (GPhC premises registration number 2205750). Our superintendent pharmacist is Blaal Hussain.

We are the data controller for the personal information you give us through this website: we decide how it is used, and we are responsible for keeping it safe. We provide NHS pharmacy services in Scotland under arrangements with our NHS board.

This website and our pharmacy records system are provided by The Chemassist. The Chemassist is our data processor: it handles your information only on our instructions, under a written contract, and may not use it for its own purposes.

What we collect

  • Your details: name, date of birth, address and postcode, phone number and email address.
  • Health information you give us for a service: symptoms, medical history, medicines, allergies, your answers to the pharmacist’s questions, and your GP practice.
  • Prescription requests.
  • Appointment bookings and queue check-ins: the service, date and time.
  • Messages you send us through the website.
  • Basic technical information that our servers record to keep the service secure, such as your IP address and the time of your visit.

Health information is “special category” data under UK data protection law, and we protect it accordingly.

How we use it

  • To provide the service you ask for — for example NHS Pharmacy First Scotland, a consultation, a repeat prescription or an appointment — and to contact you about it.
  • To keep an accurate record of the care we give you, as the law and our NHS contract require.
  • To share information with your GP practice and others caring for you when that is needed for your care (see “Who we share it with”).
  • To claim payment for NHS services, and for the checks and audits NHS Scotland requires.

We do not sell your information or use it for advertising, and this website does not use analytics or tracking.

Our lawful basis

Under the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018:

  • NHS services: Article 6(1)(e), a task carried out in the public interest, and Article 9(2)(h), the provision of health care.
  • Private services: Article 6(1)(b), our contract with you, and Article 9(2)(h), the provision of health care.
  • Records we must keep by law: Article 6(1)(c), a legal obligation.

Because we rely on these bases, we do not need your consent to use your information for your care. If we ever ask for your consent for something else, you can withdraw it at any time.

Who we share it with

  • Your GP practice — for example a notification after a consultation, or a prescription request we send to your surgery for you.
  • Other health professionals involved in your care, such as an out-of-hours service or a hospital, when that is needed for your care.
  • NHS Scotland bodies, where our NHS contract or the law requires it — for example NHS National Services Scotland, which processes payment for NHS pharmacy services, and your NHS board.
  • Regulators, the police or the courts, only when the law requires us to.

Service providers that work for us, under contract and only on our instructions:

  • The Chemassist, which provides and hosts this website and our records system.
  • Resend, which sends our emails, such as booking confirmations.
  • getAddress.io, which finds your address from the postcode you type (it receives the postcode only).

The map on our Find us page loads map images from OpenStreetMap, whose servers receive your IP address when you view the map. The directions links open Google Maps or Apple Maps.

Transfers outside the UK

Some of our service providers (Resend) are based in the United States. When your information is processed outside the UK, it is protected by UK adequacy regulations or by the UK’s approved data transfer contracts.

How long we keep it

We keep pharmacy records for as long as NHS Scotland requires, following the Scottish Government’s Records Management Code of Practice for Health and Social Care, and then delete them securely.

Information kept on your own device, such as a form you have not finished, is deleted when you submit it or after 7 days — see our Cookie Policy.

Questions about your information

We do not have a separate Data Protection Officer. Our superintendent pharmacist, Blaal Hussain, is responsible for how we look after your information — please contact them using the details below.

Your rights

You have the right to:

  • ask for a copy of the information we hold about you;
  • ask us to correct it;
  • ask us to delete it or restrict how we use it — although we may have to keep health records by law;
  • object to how we use it;
  • where it applies, receive it in a format you can take elsewhere.

To make a request, contact us using the details below. We will reply within one month. In Scotland, a young person aged 12 or over is presumed to be able to make these requests for themselves.

Complaints

If you are unhappy with how we have handled your information, please contact us first. You can also complain to the Information Commissioner’s Office (ICO): ico.org.uk, telephone 0303 123 1113.
